Epigenetics of rheumatoid arthritis

It's not just our DNA that makes us susceptible to disease and influences its impact and outcome. Scientists are beginning to realize more and more that important changes in genes that are unrelated to changes in the DNA sequence itself ? a field of study known as epigenetics ? are equally influential.

A research team at the University of California, San Diego ? led by Gary S. Firestein, professor in the Division of Rheumatology, Allergy and Immunology at UC San Diego School of Medicine ? investigated a mechanism usually implicated in cancer and in fetal development, called DNA methylation, in the progression of rheumatoid arthritis (RA). They found that epigenetic changes due to methylation play a key role in altering genes that could potentially contribute to inflammation and joint damage. Their study is currently published in the online edition of the Annals of the Rheumatic Diseases.

"Genomics has rapidly advanced our understanding of susceptibility and severity of rheumatoid arthritis," said Firestein. "While many genetic associations have been described in this disease, we also know that if one identical twin develops RA that the other twin only has a 12 to 15 percent chance of also getting the disease. This suggests that other factors are at play ? epigenetic influences."

DNA methylation is one example of epigenetic change, in which a strand of DNA is modified after it is duplicated by adding a methyl to any cytosine molecule (C) ? one of the 4 main bases of DNA. This is one of the methods used to regulate gene expression, and is often abnormal in cancers and plays a role in organ development.

While DNA methylation of individual genes has been explored in autoimmune diseases, this study represents a genome-wide evaluation of the process in fibroblast-like synoviocytes (FLS), isolated from the site of the disease in RA. FLS are cells that interact with the immune cells in RA, an inflammatory disease in the joints that damages cartilage, bone and soft tissues of the joint.

In this study, scientists isolated and evaluated genomic DNA from 28 cell lines. They looked at DNA methylation patterns in RA FLS and compared them with FLS derived from normal individuals or patients with non-inflammatory joint disease. The data showed that the FLS in RA display a DNA methylome signature that distinguishes them from osteoarthritis and normal FLS. These FLS possess differentially methylated (DM) genes that are critical to cell trafficking, inflammation and cell?extracellular matrix interactions.

"We found that hypomethylation of individual genes was associated with increased gene expression and occurred in multiple pathways critical to inflammatory responses," said Firestein, adding that this led to their conclusion: Differentially methylated genes can alter FLS gene expression and contribute to the pathogenesis of RA.

FACT CHECK: Buyer beware in health debate

WASHINGTON (AP) ? President Barack Obama promises nothing will change for people who like their health coverage except it'll become more affordable, but the facts don't back him up. Mitt Romney groundlessly calls the health care law a slayer of jobs certain to deepen the national debt.

Welcome to the health care debate 2.0. As the claims fly, buyer beware.

After the Supreme Court upheld the law last week, Obama stepped forward to tell Americans what good will come from it. Romney was quick to lay out the harm. But some of the evidence they gave to the court of public opinion was suspect.

A look at their claims and how they compare with the facts:

___

OBAMA: "If you're one of the more than 250 million Americans who already have health insurance, you will keep your health insurance. This law will only make it more secure and more affordable."

ROMNEY: "Obamacare also means that for up to 20 million Americans, they will lose the insurance they currently have, the insurance that they like and they want to keep."

THE FACTS: Nothing in the law ensures that people happy with their policies now can keep them. Employers will continue to have the right to modify coverage or even drop it, and some are expected to do so as more insurance alternatives become available to the population under the law. Nor is there any guarantee that coverage will become cheaper, despite the subsidies many people will get.

Americans may well end up feeling more secure about their ability to obtain and keep coverage once insurance companies can no longer deny, terminate or charge more for coverage for those in poor health. But particular health insurance plans will have no guarantee of ironclad security. Much can change, including the cost.

The non-partisan Congressional Budget Office has estimated that the number of workers getting employer-based coverage could drop by several million, as some workers choose new plans in the marketplace or as employers drop coverage altogether. Companies with more than 50 workers would have to pay a fine for terminating insurance, but in some cases that would be cost-effective for them.

Obama's soothing words for those who are content with their current coverage have been heard before, rendered with different degrees of accuracy. He's said nothing in the law requires people to change their plans, true enough. But the law does not guarantee the status quo for anyone, either.

So where does Romney come up with 20 million at risk of losing their current plans?

He does so by going with the worst-case scenario in the budget office's analysis. Researchers thought it most likely that employer coverage would decline by 3 to 5 million, but the range of possibilities was broad: It could go up by as much as 3 million or down by as much as 20 million.

ROMNEY: After saying the new law cuts Medicare by $500 billion and raises taxes by a like amount, adds: "And even with those cuts and tax increases, Obamacare adds trillions to our deficits and to our national debt, and pushes those obligations onto coming generations."

THE FACTS: In its most recent complete estimate, in March 2011, the Congressional Budget Office said the new health care law would actually reduce the federal budget deficit by $210 billion over the next 10 years. In the following decade, the law would continue to reduce deficits by about one-half of one percent of the nation's gross domestic product, the office said.

The congressional budget scorekeepers acknowledged their projections are "quite uncertain" because of the complexity of the issue and the assumptions involved, which include the assumption that all aspects of the law are implemented as written. But the CBO assessment offers no backup for Romney's claim that the law "adds trillions to our deficits."

OBAMA: "And by this August, nearly 13 million of you will receive a rebate from your insurance company because it spent too much on things like administrative costs and CEO bonuses and not enough on your health care."

THE FACTS: Rebates are coming, but not nearly that many Americans are likely to get those checks and for many of those who do, the amount will be decidedly modest.

The government acknowledges it does not know how many households will see rebates in August from a provision of the law that makes insurance companies give back excess money spent on overhead instead of health care delivery. Altogether, the rebates that go out will benefit nearly 13 million people. But most of the benefit will be indirect, going to employers because they cover most of the cost of insurance provided in the workplace.

Employers can plow all the rebate money, including the workers' share, back into the company's health plan, or pass along part of it.

The government says some 4 million people who are due rebates live in households that purchased coverage directly from an insurance company, not through an employer, and experts say those households are the most likely to get a rebate check directly.

The government says the rebates have an average value of $151 per household. But employers, who typically pay 70 to 80 percent of premiums, are likely to get most of that.

ROMNEY: "Obamacare raises taxes on the American people by approximately $500 billion."

THE FACTS: The tax increases fall heavily on upper-income people, health insurance companies, drug makers and medical device manufacturers.

People who fail to obtain health insurance as required by the law will face a tax penalty, although that's expected to hit relatively few because the vast majority of Americans have insurance and many who don't will end up getting it. Also, a 10 percent tax has been imposed on tanning bed use as part of the health care law. There are no other across-the-board tax increases in the law, although some tax benefits such as flexible savings accounts are scaled back. Of course, higher taxes on businesses can be passed on to the consumer in the form of higher prices.

Individuals making over $200,000 and couples making over $250,000 will pay 0.9 percent more in Medicare payroll tax and a 3.8 percent tax on investments. As well, a tax starts in 2018 on high-value insurance plans.

OBAMA: "Because of the Affordable Care Act, young adults under the age of 26 are able to stay on their parents' health care plans, a provision that's already helped 6 million young Americans."

THE FACTS: Obama is overstating this benefit of his health law, and his own administration knows better. The Department of Health and Human Services, in a June 19 news release, said 3.1 million young adults would be uninsured were it not for the new law. Obama's number comes from a June 8 survey by the Commonwealth Fund, a health policy foundation. It said 6.6 million young adults joined or stayed on their parents' health plans who wouldn't have been able to absent the law. But that number includes some who switched to their parents' plans from other coverage, Commonwealth Fund officials told the Los Angeles Times.

ROMNEY: "Obamacare is a job-killer."

THE FACTS: The CBO estimated in 2010 that the law would reduce the amount of labor used in the economy by roughly half a percent.

But that's mostly because the law will give many people the opportunity to retire, stay at home with family or switch to part-time work, since they will be able to get health insurance more easily outside of their jobs. That voluntary retreat from the workforce, made possible by the law's benefits, is not the same as employers slashing jobs because of the law's costs, as Romney implies.

The law's penalties on employers who don't provide health insurance might cause some companies to hire fewer low-wage workers or to hire more part-timers instead of full-time employees, the budget office said. But the main consequence would still be from more people choosing not to work.

Apart from the budget office and other disinterested parties that study the law, each side in the debate uses research sponsored by interest groups, often slanted, to buttress its case. Romney cites a Chamber of Commerce online survey in which nearly three-quarters of respondents said the law would dampen their hiring.

The chamber is a strong opponent of the law, having run ads against it. Its poll was conducted unscientifically and is therefore not a valid measure of business opinion.

True nature of staff motivation more complex than surveys reveal

ScienceDaily (July 3, 2012) ? High levels of staff engagement could actually be damaging for organisations if overly simplistic staff surveys mask the type of engagement at play within an organisation, according to new research from Kingston University's Business School and the Chartered Institute of Personnel and Development (CIPD).

The study found a key difference between types of employees. Those who are involved only with the task at hand (known as transactionally engaged) tend to respond positively to staff surveys, but are often likely to leave quickly for a better offer. However, those who are very positive and feel strongly about the organisation's mission and values (dubbed emotionally engaged) are more likely to perform at a higher level and remain committed through good times and bad.

"The research has identified the clear difference between the people who are primarily working just to make money and have no specific feelings and ties to their employer, compared with those staff who have a closer emotional attachment to their employers," Stephen Gourlay, Deputy Director of Kingston Business School's Centre for Research in Employment, Skills and Society (CRESS) said. "The former group are likely to perform their job reasonably well, but the latter will often go out of their way to carry out additional tasks."

The researchers identified so-called transactional engagement as being shaped simply by employees' concern to earn a living and to meet the most minimal expectations of their employers. Emotional engagement, meanwhile, is influenced by different aspects of work that go beyond the job role itself, including relations with colleagues, line managers, the organisation and clients or customers. It is driven by a desire on the part of employees to do more for the organisation than is normally expected and in return to receive a more fulfilling psychological contract, i.e. to feel more valued.

High numbers of staff displaying transactional engagement were found to be potentially damaging for both individuals and the organisations they work for. This kind of employee (i.e. those just working for the money) reported higher levels of stress and difficulties in achieving a work-life balance than those employees who were emotionally engaged. They were also found to be more likely to indulge in behaviour which could actually damage the organisation than their emotionally engaged counterparts.

"Our research reveals that this whole area of engagement is much more complex than has been portrayed in the past," Dr Gourlay added. "Now further research is needed to examine this more thoroughly."

Among all the Windows 8, WP8 and Surface excitement of late, Redmond has also dropped a chunk of less shiny happy news: its online services division is taking a goodwill writeoff of $6.2 billion as a result of its ill-fated aQuantive acquisition in 2007. Not coincidentally, that's almost exactly what it paid for the company, which it brought in to create pre-Bing online ad revenue -- back when Mountain View was eating even more of its lunch in search. The software giant said that aQuantive didn't "accelerate growth" as much as intended, although it added that it still provides assets for its internet advertising activities. With the advent of tablets and smartphones since then, it seems unlikely we'll see any of its now-quaint tech -- like shopping cart-mounted computers -- again.

Fake Craigslist ad for nude photos was 'prank'

lt was meant as a "prank,"?but it wound up terrifying the victim: A male "friend" placed a Craigslist ad in a woman's name, along with graphic photos of her, inviting men to respond in kind and?send porn photos of themselves to the woman.

The woman, a resident of Chesterfield Township, Michigan, received more than 80 text messages within the first hour the ad was placed online, she told police.

"The person who posted it thought it would be funny, and didn't think it would get that many responses," Detective Sergeant Deron Myers of the Chesterfield Police told msnbc.com.

After the victim contacted police, they got a search warrant for Craigslist?to find out the name of the person who put the ad online. When the victim learned it was a friend of hers, she declined to press charges, Myers said. And the "friend" was appropriately remorseful, he said.

"It got way more attention than he thought" it would, Myers said. In the end,?"All parties just kind of shrugged it off as a tasteless joke."

Not all such incidents work out that way. Last year, Tracy Wilder, of Florida,?started getting knocks on her door, emails and text messages after the wife of Wilder's ex-husband placed a steamy?Craigslist ad in Wilder's name, giving out her phone number and inviting men to come to Wilder's home, with children living there.

The experience was "absolutely appalling,"?Wilder told TODAY's Ann Curry, last summer. (That video interview is below.)

Bobby Womack makes his return with 'Bravest Man'

NASHVILLE, Tenn. (AP) ? Bobby Womack has been frustrated.

He's relaunching his career after what amounts to a two-decade, self-imposed exile and he can't get his doctor to cooperate. There's so much to do, Womack says in a gravelly voice, which has aged like the most expensive single-malt whiskey. Yet his doctor has ordered him to shut down for nearly two weeks ? an eternity for a restless man seeking a rare second act at age 68.

"Can I tell you the honest truth? I've been through a lot, more than I've ever been through in my life, in the last two months," Womack says in a telephone interview from his Los Angeles home. "You know, I had prostate cancer, then they got rid of that and discovered I had colon cancer. Then after that my lungs completely shut down and they had to put me on a machine and I was out in a coma for 10 days. Then after that I had walking pneumonia ? twice. So there's only so much the body can take. I want to go back to work, but the doctor says, 'Man, you've got to rest 10 days and not do NOTHING.' He said, 'Because we done lost you,' and he said, 'It's a miracle you're walking around.'"

And Womack can't disagree, especially when you take the long view.

A gospel and soul singer, a songwriter and a guitar player with few peers, Womack is largely a forgotten figure today ? but one who is getting a close re-examination. His new album, "The Bravest Man in the Universe," was produced by Damon Albarn, and Womack has ambitious plans for a tour later this year ? if his body and doctor cooperate.

His return is a welcome development for those who recognize his deep contributions to modern music ? as Sam Cooke's guitarist, the writer of the Rolling Stones' first hit and someone who scored many of his own during his solo career. Some worried Womack might not have the same vitality that made him one of the most inspiring and imitated artists of his generation. And even Womack wondered sometimes. He admits to having seizures during his tour with Albarn's Gorillaz several years ago and his health problems loomed large.

"Yet as so often is the case with Bobby," said Mark Rowland, executive producer of the "Unsung" series, "he gets counted out over and over in different ways throughout his career, and yet he keeps coming back."

The "Unsung" episode that aired in January on TV One featuring Womack was among the most watched and commented upon in the series.

"There's so much to say about his music and there's so much to say about his life," Rowland said. "He's had a life that's really even more dramatic than some of the more dramatic aspects of the songs he's written."

Womack hopes for a little drama in the next chapter of his career and calls "Bravest Man" ''a fresh start." It's his first collection of original material in 18 years ? since he quit the life to save his life. He gave up music because he was addicted to drugs and alcohol. Drugs and music. Music and drugs. The two were so intrinsically entwined, he felt the only way to really quit one was to give up the other.

He released a few albums of others' material during this hiatus, but it wasn't until Albarn tapped him for his Gorillaz side project that Womack caught the fever again. The way he sees it, he's carrying the torch for all those friends who fell victim to the lifestyle or were forgotten by time.

"I've got to represent some of the greatest soul singers that ever walked in life. I still feel that they don't get their propers for what really happened," he said. "Since I've outlived 90 percent of those people, I said, 'Let me make a statement.'"

"The Bravest Man in the Universe" is a broadside at soul traditionalists looking for a recreation of something Womack might have dropped in the 1960s or '70s at the height of his influence. Programmed beats and electronic sounds skitter behind Womack's voice and a smattering of traditional instruments, including piano and Womack's guitar.

Richard Russell, Albarn's co-producer on "Bravest Man," thinks their work fits right into the spirit of Womack's career. He calls it "organic and earthy."

"I didn't see what we were doing being in any way any type of radical departure from what he's ever done before, and I've been quite surprised at some of the feedback to it which has suggested that it is that," Russell said in a phone interview from London. "Bobby's always made rhythmic records that have always been modern at the point he's been doing them, right? Bobby's never made retro records and there's no reason why he should now."

Womack entered the 2011 recording sessions for the album with an open mind and knew they'd be using programmed beats. But he admitted the recording style employed by Albarn, the Blur frontman, and Russell, the XL Recordings owner, took some getting used to.

The trio sat down together in the round, joined by Womack's writing partner Harold Payne, and hammered out arrangements as they went. Womack worried they were moving too quickly, but says he loved the final product.

"As loooong as the most dominant thing is there and that's your vocals and your lyrics ? that's your story ? that's all that matters," Womack said. "Ain't nobody going to be listening to who programmed the drums or who did the piano. They don't care. They're just listening to the basic song itself. And when you hear me sing, even if you've only heard me sing once, you know my voice."

China HSBC PMI hits seven-month low of 48.2 in June

BEIJING (Reuters) - China's factory activity shrank in June at the fastest pace in seven months as new export orders tumbled to depths last seen in March 2009, a private sector survey showed, underlining the risk of a lurch lower for the Chinese economy.

The HSBC Purchasing Managers' Index (PMI) fell to 48.2 after seasonal adjustments, its lowest since November 2011, and little changed from a flash, or preliminary, estimate of 48.1. The final reading in May was 48.4.

June was the eighth straight month of a reading below 50, the threshold dividing expansion from contraction in the survey methodology.

China's official PMI, released on Sunday, also fell to a seven-month low in June. However, the official PMI was 50.2, indicating the sector was still expanding.

The two indexes often give divergent readings as the official PMI surveys mainly big, state-backed firms, while the HSBC PMI takes the pulse of more smaller, private-sector companies.

But both readings underline speculation that Beijing will relax monetary and fiscal policies further to boost domestic activity to compensate for weakening foreign demand for China's factory goods - a problem for the world's biggest exporter.

The survey said export demand from Europe and North America was especially weak.

"We expect more decisive easing efforts to come through in coming months," said Qu Hongbin, an economist at HSBC.

"It is all about growth and employment," Qu said. "As external demand has weakened and domestic demand hasn't shown a meaningful improvement in response to earlier easing measures, growth is likely to be on track for further slowdown."

Like the official PMI, the HSBC PMI showed new orders and new export orders struggling in June below the 50-point threshold, suggesting demand was shrinking.

The new export orders sub-index lost a hefty 2.7 points to sink to a 39-month low of 45.8.

The new orders sub-index also fell, although not as sharply as new export orders. But the combined drop in export and domestic orders was the biggest so far this year, the survey said.

Sliding production cooled prices. Average production costs fell at their steepest rate since March 2009. Prices of finished goods fell at the fastest pace in 42 months, although in part due to competitive pressures between producers.

Crucially, it said there were also signs that factories were saddled with excess capacity as backlogs of work fell slightly as factories received fewer new orders.

Factory employment shrank for the fourth straight month in June, though the employment sub-index rebounded to a three-month high from May's 38-month low.

Job creation is a key indicator for China's government, which calibrates policy to ensure growth is fast enough for the Communist Party to make good on pledges to raise incomes for the poor and achieve social stability, thereby justifying its grip on power.

The loss of around 20 million Chinese jobs in a matter of months towards the end of 2008 during the depths of the global financial crisis, as international trade ground to a halt and factory activity tumbled, prompted Beijing to fashion a 4 trillion yuan ($635 billion) stimulus program in response.

To shore up the economy this time around, Beijing has lowered interest rates once and reduced banks' reserve requirement ratio twice this year. Investors expect to see more, though few anticipate a full-blown fiscal stimulus package.

A Reuters poll in May showed analysts expect China to lower banks' reserve requirements by another 100 basis points this year to 19 percent for its biggest banks.

The full PMI report, compiled by British research firm Markit, also highlighted the following findings:

- Factories surveyed said demand was muted for both domestic and foreign markets. Demand was especially weak in Europe and North America.

- Around 15 percent of factories polled said new export business fell in June, compared to 6 percent of respondents who said business improved.

- Inventories of finished goods rose, and anecdotal evidence suggested the increase was a result of unexpectedly weak sales. Inventories rose at their second-fastest rate since records began in April 2004, although the survey said the speed of increase was still modest.

- Falling commodity and crude oil prices dragged input prices down to their second-lowest level on record, behind all-time lows seen during the 2008/09 crisis.

- Slackening demand for raw materials improved the delivery times of suppliers.

- Purchases of raw materials fell due to weak demand, and as factories tried to run down their stocks.
